Dr. Dave Coulson
Barnet Dulaney Perkins Eye Center
Dr. David Coulson received his Doctorate of Optometry degree from Pacific University College of Optometry in Forest Grove, Oregon in 2007 and his Bachelor of Science, Kinesiology degree from the Arizona State University in Tempe, Arizona, graduating Magna Cum Laude in 2003. He completed his training by working as an intern for Barnet Dulaney Perkins Eye Center. Prior to joining Barnet Dulaney Perkins, Dr. Coulson volunteered his time at the Southwest Washington Free Clinic and the Native American Rehabilitation Association Diabetic Clinic providing free eye exams low income patients and patients with Diabetes Mellitus. He is an active member of the Arizona Optometric Association and the American Optometric Association. Dr. Coulson was also the winner of the coveted 2011 Young Optometrist of the Year award.
Dr. Beth Pyle-Smith

Dr. Beth Pyle-Smith started practicing in Arizona in 1993 after she graduated from optometry school. She worked for an HMO in Phoenix for 8 years prior to joining Drs. A. James Frank and Forrest Hill's practice in 2000. She went with Dr. Frank to Guatemala on a Mission Trip in 2000. She began working with Dr Frank at the Phoenix office and then transitioned to their new office in Avondale. She enjoys practicing full-scope optometry, from examining infants all the way up to seniors. She strives to serve her patients to the best of her ability and to make sure you understand your eye conditions. She grew up in Southern California and moved out to AZ for a job opportunity. Now, she enjoys the wonderful weather here but misses the cool ocean breezes. She is married and has two children. Her family loves to raise and train Dobermans and show them in dog shows around the valley. In her spare time, she likes to... wait she has small kids... she has no spare time!
Dr. Jon Wold
San Tan Eye Care
Following a 2 year church service mission to Romania, Dr. Wold received his Bachelor of Science degree from Arizona State University in 1999 and graduated magna cum laude from the University of Houston College of Optometry in 2003. He completed his externship at Barnet Dulaney Perkins Eye Center in Mesa, AZ and Indian Health Hospital in Phoenix, AZ. Dr. Wold is actively involved in Boy Scouts of America and is a member of the American Optometric Association and the Arizona Optometric Association. Dr. Wold married his wife, Emily Taylor, in 1996 and they have five children: Allison, Adam, Nathan, Ryan, and Katie. In his spare time, he enjoys going to concerts, traveling with his wife, basketball with his boys, biking, hiking, and spending time with family.
Dr. Leslie Falcon

Dr. Falcon graduated With Honors from the Michigan College of Optometry in May of 2005. Her training included externships in the areas of pediatric Optometry, advanced care for the contact lens patient and ocular disease management. In 2012 Dr. Falcon was awarded the coveted Young Optometrist of the Year by the Arizona Optometric Association. Dr. Falcon served as an Arizona delegate to the American Optometric Association Congressional Advocacy Conference.
Dr. Todd Smith

Dr. Smith was born at Good Samaritan Hospital in Phoenix Arizona and finished grammar school in Colorado Springs, Colorado. He graduated with honors in 1998 from the Arizona State University with a Bachelors Degree in Biology. He had several rotations while attending The Southern California College of Optometry including an ocular disease rotation at the Veterans Affairs Hospital in Amarillo Texas, Macalapa Medical Clinic in Pearl Harbor Hawaii, and the Barnet Dulaney Perkins Eye Center in Phoenix Arizona. Dr. Smith worked with Dr. Mel S. Moss in Phoenix until 2006, and currently owns and works at The Westbrook Vision Center. He currently sees patients and lectures on medial billing and coding at Midwestern University several days a week. Dr. Smith is a member of The American Optometric Association and a director for The Arizona Optometric Association. He lives in Northwest Peoria with his wife Bethanie, son Ethan, daughter Madeline and rotten dog Gizmo.
Dr. Lindsey Clyde

Earning her Bachelor degree in Biology, Dr. Lindsey Clyde graduated from University of Colorado, near her hometown of Colorado Springs. She then received her Doctorate from NOVA Southeastern University College of Optometry in Fort Lauderdale. While there, she completed an externship in Ocular Disease at The Bascom Palmer Eye Institute, before moving back to her home state to practice for one year. Dr. Clyde was anxious to get back to sunny weather, so relocating to Arizona in 2006 was the perfect move for her. She now owns a dual-office private practice, which continues to thrive. When she's not seeing patients, Dr. Clyde and her husband Greg enjoy traveling as much as they enjoy staying in and playing with their two shih tzus Kevin & Brian and Bill the Cat.
Dr. Justin Jones

Dr. Justin Jones received a Bachelor degree in Biology and in Vision Science and his Doctorate of Optometry degree with Honors from Northeastern State University College of Optometry in Oklahoma. Dr. Jones completed an ocular disease internship at Hu Hu Kam Memorial Hospital in 2005. Dr. Jones is active in the Arizona Optometric Association, Contact Lens and Cornea Section, and the AOA Political Action committee. Dr. Jones has been a resident of Arizona since 2005. Dr. Jones is married to, Darcy, who is also an optometrist. She works with the Gila River Tribe. Both Justin and Darcy are originally from Oklahoma, but they are now glad to call Phoenix home!
Dr. Armanae Mancha

Dr. Mancha was born in Odessa, Texas. She received a Bachelor of Science degree in Biology Summa Cum Laude from the University of Texas. She earned her Doctorate of Optometry degree Magna Cum Laude from the University of Houston College of Optometry in 2003. Dr. Mancha’s clinical interests include specialty contact lens fittings, especially multifocal lenses, as well as pediatric optometry. She is an InfantSEE provider, offering comprehensive eye examinations to infants starting at 6 months of age. Dr. Mancha is also extensively trained in vision rehabilitation and the management of ocular disease. In addition, she is certified to perform Corneal Refractive Therapy/Orthokeratalogy. Dr. Mancha is currently an active member of the Arizona Optometric Association and the American Optometric Association. She is fluent in Spanish and looking forward to providing exceptional eye care to the Scottsdale and Phoenix community.
Dr. Bart Pemberton, FAAO

Dr. Pemberton received his BA in Biology from the University of California at Riverside in 1991 and his Doctor of Optometry degree from the University of California at Berkeley in 1995. He completed a one-year residency in Hospital-based Optometry at the VA Medical Center in Amarillo, TX, in 1996. He has practiced in Tucson, AZ since 1997, and he has practiced with Drs. Edmonds and Husz since 1999. He is a past president of the Southern Arizona Optometric Society, a Fellow of the American Academy of Optometry, and a member of the American and Arizona Optometric Associations. He has particpated in several studies for the contact lens industry. He, along with Dr. Edmonds and Dr. Husz, is an adjunct clinical professor for the Pacific University College of Optometry in Forest Grove, OR.
Originally from Lockport, N.Y. he attended Erie Community College in Buffalo and received an AAS degree in Ophthalmic Dispensing and attended the University of Nevada – Las Vegas for two years before moving to Memphis and graduating from the Southern College of Optometry in 1984. Dr. Meier has held leadership roles in local, state and regional Optometric associations and was appointed to the Arizona Board of Optometry by then Governor Janet Napolitano. Along with having a private optometry practice in Phoenix, he has served as the AZOA Executive Director since 2011.
